Getting there depends on three interconnected things: accurate testing to confirm the diagnosis, consistent monitoring of how well your lungs are working, and a clear written plan that tells you what to do when symptoms shift. This guide walks through each of those pillars in plain language.

How Asthma Is Diagnosed

No single test diagnoses asthma on its own. Clinicians typically piece together a picture using your symptom history, physical exam findings, and one or more lung function tests. The hallmark of asthma is variable airflow limitation — meaning the airways narrow and widen over time, often in response to triggers like cold air, exercise, allergens, or respiratory infections.

If a clinician suspects asthma, they will usually ask about the pattern of your symptoms. Wheezing, chest tightness, shortness of breath, and a persistent cough — especially at night or early in the morning — are classic signs. A family history of asthma or allergic conditions can also raise suspicion.

Key Lung Function Tests

Breathing tests give clinicians objective data beyond what symptoms alone can reveal. Here are the most commonly used ones:

  • Spirometry: This is the standard first-line test for asthma. You breathe as hard and fast as you can into a device called a spirometer, which measures how much air you move and how quickly you move it. Two numbers matter most: FEV1 (the volume of air you can force out in one second) and FVC (the total volume you exhale). A reduced ratio between these two suggests the airways are obstructed.   • Exhaled nitric oxide (FeNO) testing: You breathe out slowly into a device that measures levels of nitric oxide in your breath. Elevated levels can indicate eosinophilic (allergic-type) airway inflammation, which is a common feature of asthma. This test helps clinicians determine what type of inflammation is present and guide treatment choices.
  • Allergy testing: Identifying specific triggers through skin-prick or blood allergy tests does not diagnose asthma directly, but it helps personalize the management plan — especially for people whose asthma is driven by allergen exposure.

Understanding Oxygen Readings

You may have had a small clip placed on your finger during a doctor's visit or hospital stay. That device is called a pulse oximeter, and it estimates the percentage of hemoglobin in your blood that is carrying oxygen — a value called oxygen saturation, or SpO2.

For most healthy adults, an SpO2 reading in the mid-to-upper 90s is considered normal. During an asthma flare, narrowed airways make it harder to get enough air in, and oxygen saturation can drop. Clinicians use this reading, alongside symptoms and peak flow measurements, to gauge the severity of an episode.

Important: Home pulse oximeters became widely available during the COVID-19 pandemic, and some people with asthma now own one. While they can provide useful context, they are not a substitute for medical evaluation. If your reading drops noticeably below your usual level, or if you are experiencing significant breathing difficulty, contact a clinician or seek emergency care rather than relying solely on the device.

It is also worth knowing that pulse oximeters have limitations. Research has found that they can be less accurate in people with darker skin tones, and factors like nail polish, poor circulation, or movement can affect readings. Clinicians are aware of these limitations and may use additional measures — such as arterial blood gas testing — when more precise information is needed.

The Written Asthma Action Plan

A written asthma action plan is one of the most effective tools in asthma management, and it is consistently recommended by lung health guidelines. Yet many people with asthma do not have one. If you fall into that group, asking your clinician to create one with you is one of the highest-impact steps you can take.

Action plans are typically organized using a traffic-light framework:

  • Green zone (doing well): Your symptoms are minimal or absent, you are sleeping through the night, and your peak flow is in a healthy range relative to your personal best. You take your controller medications as prescribed and go about your normal activities.
  • Yellow zone (getting worse): You notice increased coughing, wheezing, or chest tightness, your sleep is being disrupted, or your peak flow has dipped into a moderate range. The plan outlines specific steps — often adding or increasing a quick-relief medication — and sets a timeframe for reassessing. It also specifies when to call your clinician.
  • Red zone (medical alert): Symptoms are severe, your quick-relief inhaler is not helping, or your peak flow is very low. The plan directs you to take immediate action and call emergency services or go to an emergency department without delay.

The specific medications, doses, and peak flow thresholds in your plan are individualized — which is why the plan must be developed with your own clinician rather than borrowed from a generic template. Make sure your plan is reviewed and updated at least once a year, or sooner if your treatment changes.

Controller vs. Quick-Relief Medications

A brief word on the two main categories of asthma medication, because understanding the difference is essential for using an action plan correctly.

  • Controller medications (sometimes called preventer medications) are taken daily, even when you feel fine. They work by reducing inflammation in the airways over time, making flares less frequent and less severe. Inhaled corticosteroids are the most commonly prescribed type. Their benefits are cumulative — skipping doses undermines the protection they build.
  • Quick-relief medications (also called rescue inhalers) work rapidly to open narrowed airways during a flare or before known triggers like exercise. They are not a substitute for controller therapy. Needing your rescue inhaler frequently is a signal that your asthma is not well controlled and that your overall treatment plan may need adjustment.

Identifying and Reducing Triggers

Asthma triggers vary considerably from person to person. Common ones include tobacco smoke, pet dander, dust mites, mold, outdoor air pollution, strong scents, cold air, exercise, and respiratory viruses. Keeping a simple symptom diary can help you and your clinician spot patterns. Once triggers are identified, reducing exposure — where possible — is one of the most straightforward ways to improve day-to-day control.

For people whose asthma is significantly driven by allergies, treatments such as allergen immunotherapy may be worth discussing with a specialist. For those with more severe or hard-to-control asthma, newer biologic medications that target specific inflammatory pathways have expanded the options available.

When to See a Specialist

A primary care clinician can diagnose and manage many cases of asthma effectively. However, a referral to a pulmonologist (lung specialist) or allergist is often appropriate if:

  • The diagnosis is uncertain
  • Symptoms remain poorly controlled despite treatment
  • You have had a severe or life-threatening asthma attack
  • You need frequent courses of oral corticosteroids
  • You are considering biologic therapy

Taking an Active Role in Your Care

Asthma management works best as a partnership. That means attending regular follow-up appointments, being honest with your clinician about how often you use your rescue inhaler and whether symptoms are interfering with sleep or daily activities, and learning the correct inhaler technique — something that makes a meaningful difference in how much medication actually reaches your airways.

If you are the parent or caregiver of a child with asthma, make sure the child's school or daycare has a copy of the action plan and access to a rescue inhaler. Schools in most states are required to have policies allowing students to carry and use inhalers, but the specifics vary.

Asthma is not something to simply endure. With the right combination of accurate testing, consistent monitoring, and a clear action plan, most people with asthma can live full, active lives with their symptoms well under control.
