Understanding what different temperature ranges mean — and what they don't — can help you stay calm, monitor symptoms carefully, and make smarter decisions about when to call a clinician.

What Is a Fever, Exactly?

Body temperature is not a single fixed number. It shifts throughout the day, typically sitting lower in the morning and rising slightly in the late afternoon. It can also change with physical activity, the time of your menstrual cycle, and even what you ate or drank recently.

That said, most clinicians and major health organizations define a fever in adults as an oral temperature at or above 100.4°F (38°C). Anything between about 99°F and 100.3°F is sometimes called a low-grade fever or a slightly elevated temperature — worth noting, but not the same as a true fever.

A fever is not a disease in itself. It is a physiological response — part of the immune system's toolkit. When the body detects an invader such as a virus or bacterium, immune cells release chemical signals that tell the brain's hypothalamus to raise the body's internal thermostat. The resulting warmth can slow the reproduction of some pathogens and help activate immune cells. In other words, a moderate fever is often the body working as intended.

How to Take an Accurate Temperature

The method matters. Different measurement sites produce slightly different readings, and knowing which you are using helps you interpret the number correctly.

  • Oral (under the tongue): A reliable method for most adults and older children. Wait at least 15–20 minutes after eating, drinking, or smoking before measuring, as these can skew the reading.
  • Rectal: Considered the most accurate measurement of core body temperature and is the recommended method for infants under three months. Rectal readings typically run slightly higher than oral ones.
  • Axillary (under the arm): Convenient but the least precise. Readings here tend to be lower than oral temperature. Use this method as a rough screen, not a definitive measurement.
  • Temporal artery (forehead) and tympanic (ear): These infrared thermometers are fast and easy, but accuracy depends heavily on correct technique. Follow the manufacturer's instructions closely, and be aware that ear readings can be affected by earwax or the angle of insertion.

Whatever method you use, write down the temperature, the method, and the time. A log of readings over hours or days gives a clinician far more useful information than a single number.

What the Numbers Generally Mean

Here is a broad framework — not a diagnostic guide, but a way to understand the landscape.

Low-Grade Elevation (roughly 99°F–100.3°F oral)

This range can reflect the normal afternoon rise in body temperature, mild dehydration, a recovering infection, or the very early stages of an illness. In most healthy adults, a temperature in this range without other symptoms typically warrants watchful waiting rather than urgent action. Stay hydrated, rest, and monitor for changes.

Mild to Moderate Fever (roughly 100.4°F–102°F oral)

This is the most common fever range seen with viral illnesses like colds, flu, and COVID-19. The body is actively mounting an immune response. For most otherwise healthy adults and children over three months, a fever in this range is uncomfortable but not inherently dangerous. Fever-reducing medications such as acetaminophen or ibuprofen can ease discomfort, but they do not treat the underlying cause. Discuss appropriate dosing with a pharmacist or clinician, especially for children.

High Fever (roughly 102°F–104°F oral)

A fever in this range warrants closer attention. It does not automatically mean something is seriously wrong — the flu, for example, can push temperatures this high — but you should monitor symptoms carefully. Watch for signs of dehydration (dry mouth, decreased urination, dizziness) and contact a clinician if the fever is persistent, if you have underlying health conditions, or if other concerning symptoms develop.

Very High Fever (above 104°F oral)

A temperature above 104°F in adults or older children is a signal to seek medical care promptly. Sustained temperatures at this level carry a higher risk of complications. Do not wait to see if it comes down on its own before calling a clinician or heading to an urgent care center.

Hyperpyrexia (above 106°F)

A temperature above 106°F is a medical emergency. This level of fever can be associated with serious infections, heat stroke, or other critical conditions that require immediate emergency care. Call 911 or go to an emergency room right away.

A Special Note on Infants and Young Children

The rules change significantly for babies and young children, and the thresholds for concern are lower.

  • Infants under 3 months: Any rectal temperature of 100.4°F (38°C) or higher should prompt an immediate call to a pediatrician or a trip to the emergency room. The immune system in very young infants is still developing, and what looks like a routine fever can sometimes indicate a serious infection that requires prompt evaluation.
  • Infants 3–6 months: A fever at or above 102°F, or any fever accompanied by unusual fussiness, difficulty breathing, a rash, or poor feeding, warrants a prompt call to your child's doctor.
  • Children 6 months and older: Follow the guidance below for warning signs, and always factor in how the child looks and acts — a child who is alert, drinking fluids, and still playing is generally less concerning than one who is unusually lethargic or inconsolable.

Febrile seizures — brief seizures triggered by a rapid rise in temperature — can occur in young children and are understandably frightening for caregivers. While they are alarming to witness, most febrile seizures in otherwise healthy children last only a short time and do not cause lasting harm. However, any seizure requires immediate medical evaluation to rule out other causes.

Warning Signs That Always Need Medical Attention

The temperature number alone is not the whole picture. The following symptoms alongside a fever — at any temperature — are reasons to contact a clinician or seek care without delay:

  • Stiff neck or severe headache
  • Sensitivity to light or difficulty looking at bright lights
  • Confusion, disorientation, or unusual drowsiness
  • Difficulty breathing or chest pain
  • A rash, especially one that appears as small reddish or purple spots or does not fade when pressed
  • Persistent vomiting that prevents you from keeping fluids down
  • Severe abdominal pain
  • Signs of dehydration: no tears when crying (in infants), very dry mouth, no urination for many hours
  • A fever that goes away for more than a day and then returns
  • Any fever lasting more than a few days without improvement
When in doubt, call. A quick phone call or message to your clinician's office — or a nurse hotline if your health plan offers one — can help you decide whether you need to be seen right away or can safely monitor at home.

Supporting the Body During a Fever

Whether or not you use fever-reducing medication, a few supportive measures can make the experience more manageable and help prevent complications.

  • Hydrate consistently. Fever increases fluid loss through sweating and faster breathing. Water, diluted juice, broth, and electrolyte solutions all count. Avoiding dehydration is one of the most important things you can do.
  • Rest. The immune system does significant repair work during sleep. Pushing through with normal activity is not helpful and may prolong recovery.
  • Dress and cover lightly. Piling on blankets to "sweat it out" is not evidence-based. Light clothing and a comfortable room temperature are more appropriate.
  • Avoid alcohol. Alcohol can contribute to dehydration and may interact with medications.
  • Use fever reducers thoughtfully. Acetaminophen and ibuprofen can bring down fever and ease discomfort, but follow dosing guidance carefully. Never give aspirin to children or teenagers with a fever, as it is linked to a rare but serious condition called Reye's syndrome.

The Bottom Line

A fever is the body doing its job — but that does not mean every fever can be managed at home.
