A key distinction clinicians often look for: Are these changes noticed primarily by the person themselves, or by people close to them? People in the early stages of some conditions may not be fully aware of the changes, while those around them notice. Both perspectives matter.

Why These Changes Happen: Common Causes Worth Knowing

Not all cognitive changes point to dementia — far from it. Many conditions that affect thinking and memory are treatable once identified. Some of the most common contributing factors include:

  • Sleep problems — Poor or insufficient sleep is consistently linked to impaired memory consolidation and attention. The brain uses sleep to clear metabolic waste and reinforce learning.
  • Mood disorders — Depression and anxiety can closely mimic cognitive impairment, affecting concentration, memory, and processing speed. This is sometimes called pseudodementia in older adults.
  • Thyroid dysfunction — An underactive thyroid can cause brain fog, slowed thinking, and memory difficulties that often resolve with treatment.
  • Nutritional deficiencies — Low levels of vitamin B12, for example, can damage the nervous system and impair cognition. This is particularly relevant for older adults and those on certain diets or medications.
  • Medication side effects — Many commonly used medications — including some antihistamines, sleep aids, and bladder medications — can affect cognition, particularly in older adults.
  • Cardiovascular risk factors — High blood pressure, poorly controlled blood sugar, and other vascular conditions can reduce blood flow to the brain over time, contributing to cognitive changes.
  • Chronic stress — Prolonged stress elevates cortisol, a hormone that, at high levels over long periods, has been shown in research to affect areas of the brain involved in memory and learning.

This list isn't exhaustive, but it illustrates an important point: many causes of cognitive change are addressable. Getting an accurate picture requires evaluation by a clinician who can look at the full picture.

When to Reach Out to a Doctor

There's no perfectly bright line, but here are some signals that it's time to make an appointment rather than wait and see:

You don't need a crisis to justify the conversation.

Many people avoid bringing up memory concerns because they fear what they might find out. But an evaluation is typically far less intimidating than it sounds. A clinician may:

  • Ask about your medical history, medications, and any recent life changes
  • Use brief standardized tests that assess memory, attention, language, and spatial reasoning — these are not pass/fail exams, but tools to establish a baseline
  • Order blood work to check for reversible causes like thyroid problems or vitamin deficiencies
  • Recommend brain imaging in some cases, depending on the clinical picture

The goal is not to deliver a diagnosis on the spot, but to gather enough information to understand what's happening and guide next steps. Even a reassuring evaluation has value — it establishes a baseline and can reduce anxiety that itself contributes to cognitive strain.

Protecting Brain Health: What the Evidence Supports

While no single habit guarantees protection against cognitive decline, a growing body of research points to several lifestyle factors that support brain health over time:

  • Physical activity — Regular aerobic exercise is one of the most consistently supported factors in brain health research. It promotes blood flow, supports the growth of new neural connections, and is associated with better cognitive outcomes in older adults.
  • Quality sleep — Prioritizing sleep isn't just about energy. During sleep, the brain consolidates memories and clears out cellular waste products that, when they accumulate, may play a role in neurodegenerative disease.
  • Social connection — Loneliness and social isolation are associated with accelerated cognitive aging in observational research. Staying connected — whether through community, family, or structured social activities — appears to be genuinely protective.
  • Mental engagement — Learning new things, reading, playing music, or engaging in cognitively demanding hobbies helps maintain what researchers call cognitive reserve — the brain's resilience in the face of age-related changes.
  • Cardiovascular health — Managing blood pressure, blood sugar, and cholesterol isn't just good for your heart. The brain's blood supply makes it highly vulnerable to vascular damage, and keeping those risk factors in check benefits both organs.
  • Diet — Research into dietary patterns and brain health is ongoing, but eating patterns rich in vegetables, whole grains, fish, nuts, and healthy fats have shown favorable associations in population-level studies.

A Note on Anxiety About Cognitive Change

It's worth naming directly: worrying about your brain health can itself impair cognition in the short term. Anxiety redirects mental resources, making it harder to concentrate and recall information — which can then amplify the worry. If you find yourself frequently monitoring your own thinking, it may help to speak with both a clinician about the physical side and a mental health professional about the anxiety piece.

Concern about cognitive health is understandable and increasingly common. The goal isn't to eliminate that concern entirely, but to channel it productively — toward earlier conversations with your care team, and toward the lifestyle choices that genuinely support a healthier brain over time.
